What exactly is cagrilintide?
Cagrilintide is a long-acting amylin analog developed by Novo Nordisk. Amylin, also known as “islet amyloid polypeptide,” is normally secreted by pancreatic beta cells alongside insulin, but for a long time it has been the “forgotten supporting actor” in weight-loss treatments.
Like GLP-1, Cagrilintide is administered via a once-weekly subcutaneous injection. Its molecule has been modified through acylation, significantly extending its half-life—which is why a once-weekly injection is possible.
However, what has the industry most excited is not Cagrilintide as a monotherapy, but its combination formulation, CagriSema—2.4 mg of Cagrilintide plus 2.4 mg of semaglutide. You can think of it as a dual-pathway weight-loss regimen combining “GLP-1 and insulin-like peptide.”
The Underlying Logic: A Second Pathway Beyond GLP-1
To understand why Cagrilintide is viewed so favorably, we must first understand what incretins do in the body.

Simply put: GLP-1 acts more like a “coach,” enforcing metabolic discipline; incretin acts more like a “referee,” directly putting the brakes on appetite. When both pathways are activated simultaneously, their effects naturally complement each other.
The Data Speaks for Itself: Why Is CagriSema So Exciting?

More importantly: While both cagrilintide monotherapy and semaglutide monotherapy are effective, the combination is not merely additive—it produces a synergistic amplification of effects. This is precisely the “1+1>2” logic that both the capital markets and the industry are discussing.
Why It’s Popular
The combined data certainly looks impressive. Following the release of the REDEFINE series of studies, the 22.7% figure immediately raised market expectations to new heights.
Patient needs have evolved. Many patients who take semaglutide experience a slowdown in weight loss toward the end of treatment or weight regain after discontinuation, creating an urgent need for a “stronger, more stable” treatment option.
Cagrilintide and CagriSema still face several practical challenges:
Side effects remain primarily gastrointestinal: the incidence of nausea, vomiting, diarrhea, and constipation is relatively high, similar to other GLP-1 agonists.
• The issue of muscle loss persists: the faster the weight loss, the higher the risk of muscle loss. Under the CagriSema regimen, a high-protein diet and resistance training are essential.
• Long-term safety data are limited: Further follow-up is needed to determine whether known risks associated with GLP-1 agonists—such as the risk of medullary thyroid carcinoma, pancreatitis, and gallbladder disease—are amplified in this combination therapy.
• Weight regain upon discontinuation remains a concern: There is currently no evidence to suggest that this treatment allows patients to “lose weight and keep it off permanently”; once lifestyle habits revert to previous patterns, weight will return.
It really works, but it’s not a cure-all for everyone.
Cagrilintide is essentially the inevitable result of the evolution of weight-loss drugs from “single-target” to “multi-target combinations.” GLP-1 opened the door, and next, researchers will explore all possible pathways that can be combined: GIP, glucagon, incretins… CagriSema is just the first step.
CagriSema may indeed be a more potent option than semaglutide. However, if you’re hoping a single injection will solve all your problems, it will most likely disappoint you—just like semaglutide does today.
The ultimate goal of weight-loss drugs has never been “how many jin to lose,” but rather “whether weight can be maintained after stopping treatment.” Cagrilintide brings us one step closer to this goal, but there’s still a long way to go.
